As of the time of writing, tourist visa extensions must be handled in person at a PIBA office. There is no fully online extension process for tourist entries. You (or your attorney on your behalf for preliminary steps) must attend the relevant PIBA branch. Waiting times at busy offices — particularly in Tel Aviv — can be significant, so plan ahead and aim to apply several weeks before your current permitted period expires.
